When a Teen Refuses Therapy: What Parents Can Do

You can see your teenager struggling. You have offered therapy. They have flatly refused. This is one of the most common conversations we have with parents of adolescents — and refusal is not the end of the road.

Understand the Refusal

Teens refuse therapy for predictable reasons. They associate it with "being the problem." They worry their parents will be told everything they say. They do not want to miss after-school hours. They feel invaded. Naming the real concern is the first step.

Start With a Parent Session

Parent-only sessions with a family therapist are useful in their own right. A therapist can coach you on how to talk to your teen, how to de-escalate conflict, and how to change the dynamics the teen is reacting to — all without the teen in the room. Many teens eventually agree to come once they see the parent is taking the work seriously.

Offer a Trial Session

"Try one session. If you hate it, you never have to go back." This framing works surprisingly often. Most teens who attend one session attend a second.

Choose the Right Therapist

A therapist who sees a lot of teens knows how to build rapport quickly and how to not sound like a parent. Fit matters more for adolescents than for almost any other population.

When Refusal Becomes a Safety Issue

If you are worried about self-harm or suicidal ideation, refusal becomes less negotiable. In that case, the conversation shifts to safety rather than fit, and involving your pediatrician or a crisis resource is appropriate.
