Why It Gets Missed

Classic ADHD stereotypes — hyperactive, disruptive, impulsive — skew male. Girls more often present with the inattentive subtype: daydreaming, disorganized, quiet, anxious. They do not act out in ways that trigger a referral. They work harder to compensate and sometimes succeed for years before the gap catches up.

Emotional intensity and trouble with rejection. Social difficulties, especially in groups. Procrastination followed by late-night cramming. Strong perfectionism.

When the Diagnosis Finally Surfaces

Middle school is when many girls' compensation strategies stop working. The executive demands ramp up, and the gap becomes visible. By high school, undiagnosed girls often present with anxiety or depression that is secondary to the underlying ADHD.
